Stephanie's office is conveniently located on Franklin Road, between Curtis and Cole Road. APPOINTMENTS: *Appointments are scheduled on Mondays, Tuesdays, Wednesdays and Thursdays. *Individual appointments are scheduled on the hour. Appointment times are flexible between 8:00 a.m. and 6:00 pm. *Requests for appointments can be made by calling 208.949.3056 or by emailing stephanie@hauserboisecounseling.com. *Once the initial appointment is set, the required paperwork can be emailed to you so that it may be filled out prior to the appointment. (The paperwork is a Word document which can be typed on.) Paperwork is also available under the forms link. *If you are unable to fill out the required paperwork prior to the appointment, please plan on coming 30 minutes before to your scheduled appointment to complete the required paperwork. INSURANCE & FEES: *All fees are paid in full at the time of our meeting. *I am an in-network provider for many Idaho insurance policies and will work with you to ensure that you receive the maximum insurance benefit to which you are entitled. *These payments will be paid directly to you from your insurance carrier. *A HICFA will be given, upon request, for ease in billing your insurance. *$150/First Session which will last 1 1/2 to 2 hours. *$100/Play Therapy Session *$90 /50-minute sessions. When you contact your insurance company, it's a good idea to ask at least these questions:
~Do I need obtain preauthorization to receive counseling services from Stephanie? If so, who do I contact to obtain the preauthorization? ~Is the coverage different if Stephanie is an out-of-network provider? ~Is there a maximum number of visits allowed per year? ~Is the provider required to submit reauthorization paperwork? If so, how often? Who does she need to contact? (Please obtain the name and number/ext. of the contact person.) |
